Please plan to arrive at our office at least 20 minutes before your scheduled appointment time. This allows you to provide us with your up-to-date insurance information or any changes in your medical history. We will also provide you with information about the Health Insurance Portability and Accountability Act (HIPAA) and request your review and signature to acknowledge your receipt of the information.

The Health Insurance Portability and Accountability Act (HIPAA) requires that we notify patients of our privacy practices. Below are two downloadable documents for you to review:
Cancellation Policy
If you are unable to keep your appointment, please contact our office as soon as possible. We require a minimum of 24 hours advance notice to avoid a $40.00 cancellation fee.
If you are coming to our office for a consultation and you or your primary care physician suspect you may need a surgery procedure, we will ask you to return to see us for the procedure. This allows us to schedule the amount of time needed to do the procedure and it allows us to contact your insurance if authorization is needed.
